Dear Parents/ Carers,
The school age immunisation team will be delivering the Human Papillomavirus (HPV) vaccine at your child’s school on 10/07/2024.
The HPV vaccine protects against many genital-based cancers and warts.
It also protects against mouth, throat, head and neck cancers caused by Human Papillomavirus.
Please use the link below to complete either the consent forms or a refusal for the vaccination.
